LifeWorks and Brian S. Hissom & Associates, PLLC are in-network with the following carriers. We are also happy to file as an out-of-network provider with other carriers.

Please check your coverage carefully by asking your insurance carrier the following questions. Then bring the answers to these questions to your first session.
- Do I have mental health benefits?
- What is my deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Patients are responsible for all fees, co-payments, and deductibles for services rendered. Please contact your insurance company for an explanation of benefits and financial obligations. Cash, check and all major credit cards accepted for payment.
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a $70 no show/cancellation fee for the missed session.
